Default problem with Rebody form for DVLA

Hi everyone,
so I'm just about to start filling out the rebody form to send off to the DVLA - but on the form there's not a seperate row for chassis and bodywork - they are both on the same row ... so what should I do - obviously I'm changing the body but keeping the chassis, so shouldn't I really need two rows for this? (see yellow section in attached pic)

any advice?

I think the slash is an "or". Hence your chassis no. is ........

You don't have a "Bodyshell" or monocoque. They are not referring to a removable body.

For my 250 GTO replica on a Porsche 944 donor I put the donor car Vin number in the Serial/ ID column as my donor car has a monocoque body and no chassis.
I also added the engine number in that column but left the others blank.

The origin column I filled in with the donor car registration for each row.
Make of component was Porsche in each case (even though Porsche seem to use quite a few parts from VW!)
