Default Filling in the V627 - Radically Altered Vehicle report

I hate filling in forms, especially ones so utterly pointless as the V627/1 but here goes.

Most of it seems straight forward but I was wondering if the make and model should be what it is now or what it is going to be called?

What about the 'other Information' and 'Additional Notes' sections - should these have more detail?

Please can people who know / care about these things check my answers before I send this off.

Hi,
I went through this a few months ago to get my DNA430 properly registered.
I also had to send photos of before, during and finished car. Finished car was all sides, engine bay and interior.
They also wanted a letter from DNA stating that the kit was supplied new.
With regards to the form, it looks good to me.
I did speak to a guy at Swansea and he was very helpful.

Did you send the photos with the form or did you do that at the inspection?

I sent them with the forms and they did not even come to inspect the car!
